Issue
I am interested in associating assessment records with treatment opportunities to ensure we have a clear view of all forms that were performed with any given prospect on each unique inquiry for care.
Environment
Kipu CRM, Roles: External 3rd Party VOB Worker, Admission Worker, Combined +, Combined (Admissions and B.D.), Super User, Limited Admissions Worker, VOB Worker, Restricted Admissions Worker, Treatment Opportunities.
Resolution
For guidance on how to associate assessment records to treatment opportunities, please refer to the steps below.
- Using the global search bar, search for your desired patient.
- Select Treatment Opportunities from the left-side panel.
- Select your desired treatment opportunity from the list to access the record.

Scroll down to the Assessments section of the Treatment Opportunity.
- In the Assessments section, select the Associate an Existing Assessment field to view a list of assessments that have been performed with the patient. Select your desired form from the list.
Important: Alternatively, users can add new assessment records to be performed directly in the treatment opportunity by using the Add an Assessment to be Answered field.
- Once you have associated the desired assessment record, the opportunity will display the assessment name, progress percentage, and created date.
- To preserve your changes, select Save at the top of the treatment opportunity.
Important: Please note that users can associate multiple assessment records with a single treatment opportunity.
