The alumni processing grid allows alumni coordinators to follow up with discharged patients who have voluntarily opted into the alumni/aftercare program.

Filtering by Discharge, Opts In/Opts Out of Alumni Aftercare

Alumni coordinators can use the alumni processing grid to filter by patients' statuses labeled as: discharged, opts in for alumni/aftercare, and opts out for alumni/aftercare. For more information on accessing the grid and modifying the grid state, click here.

  1. Alumni coordinators can filter patient status and view patient data points such as date of birth, primary phone number, and email. These data points can be used for patient outreach.

    Best Practice: Admissions coordinators can continue to reach out to patient records with a discharge status, as they have not yet confirmed to opt in/opt out of alumni/aftercare, and therefore may opt in.
  2. Alumni coordinators can also customize their alumni processing grid to allow a more tailored workflow for their specific needs.

Follow-Up Days

Alumni coordinators and admission staff can set the follow-up dates directly on the patient's record next to the patient's status. For more information on how to change default follow-up days, click here.

  1. The Alumni coordinator will access the Alumni Processing Grid, where they can find the patient they want to follow up with.

  3. The alumni coordinator can document any communication with or messages left for the client. 2025-04-09_16-04-05.pngImportant: A follow-up date can be added to the Communication Log that will prompt a task to be created, which can be used on a case-by-case basis if you need to follow up with the patient before the scheduled follow-up day filter cadence.
