Patient record statuses allow users to quickly document and view the current status of each patient in the system, which improves awareness and patient management.

Patient Record Status Explanation Associated Symbol

Prospect

A person likely to become a patient
Unknown Lead Typically refers to a potential patient seeking mental health services whose specific details or background are not yet identified or fully understood by the service provider
Lead Refers to a potential patient or individual who has expressed interest in seeking mental health or substance abuse services
Patient

Refers to an individual who is receiving, or has received, mental health or substance abuse services from a healthcare provider or treatment facility

Automatically applied to patient records when an admit date and MR number are added to the facesheet in the EMR

Client

Refers to an individual who is receiving, or has received, mental health or substance abuse services from a healthcare provider or treatment facility

Automatically applied to patient records when an admit date and MR number are added to the facesheet in the EMR

Discharged

Refers to an individual who has completed their course of treatment and has been formally released or discharged from a healthcare facility or treatment program

Automatically applied to patient records upon receipt of discharge date from EMR facesheet

Opts in for Alumni/Aftercare

An individual who has chosen to participate in post-treatment support services or aftercare/alumni programs

Opts out for Alumni/Aftercare

An individual who has chosen not to participate in post-treatment support services, including alumni or aftercare programs

Obsolete

Refers to a record or entry in the system that is outdated, no longer relevant, or has become obsolete due to changes in the patient's status
