Form Queue Processing

When a patient is created through a form queue record with an attached form template, the assessment will automatically appear in the Assessments section of the new patient record for easy access and use.

Dispositioning Form Queue Records

All form submissions made on your integrated web forms will flow to the Form Queue. Locate your desired form queue submission.

  1. Navigate to CRM > Admissions Management > Queues > Form Queue

  2. All form submissions made on your integrated web forms will flow to the Form Queue. Locate your desired submission. Select the Date Created column to open the form record.

  3. Once the form queue modal appears, select a form disposition, add any applicable notes, apply your desired form queue tags, and set the new record type if needed. Select Save to record your changes.

  4. The Form record will now be present in the Dispositioned Records view of the Form Queue.

Managing Form Queue Disposition

  1. Super users can manage the Form Queue disposition list by navigating to Admin > Preferences > Customize Lists.

  2. Select Form Queue Disposition. Set your items to yes or no to mark them visible.

Creating Patients via Form Queue

  1. Locate your desired form queue record. Select the Date Created column to access the form submission.

  3. Users will be taken to a new tab on their browser with the new patient record ready.

  4. Patient records created from the Form Queue will contain an auto-generated treatment opportunity.

Associating Form Queue Records to Existing Patients

  1. Locate your desired form queue record. Select the Date Created column to access the form submission.

  2. The form queue record modal will present all possible matching pre-existing records based on Full Name and/or Date of Birth. Additionally, users may utilize the search field to retrieve desired matches in case no matches were presented due to no matching information.

  3. Locate the matching pre-existing record. Click Select to associate the form queue record to the patient. Users may review the matching record prior to the association by selecting View.

  4. Once the pre-existing record is associated, the modal will display a yellow highlight on the associated record. Set your desired Form Queue Disposition. Select Save. If a CRM 2.0 assessment template is tied to your web form, it will populate in the Assessments section of the associated patient’s record.

Creating Referral Sources via Form Queue

  1. Locate your desired Form Queue record. Select the Date Created column to access the form submission.

  2. Review the information presented on the form submission. Select New Referral

    Important: The system will adhere to the following behavior:

    • If the form submission contains a referral source & contacts already present in the system, it will not create a new record. Form Queue record will simply be linked to the existing referral source.
    • If the form submission contains an existing referral source along with a new contact, the system will simply add the new contact to the existing referral source in the system.
    • If the form submission contains the referral source and patient information, the system will populate the referral source as the referred-through for the patient on the auto-generated treatment opportunity.
  3. The form queue record will automatically be dispositioned as Referral Added. The user will be taken to a new tab showcasing the referral source record that was generated using the mapped data points.

Associating Form Queue Records to Existing Referral Sources

  1. Locate your desired Form Queue record. Once located, select the Date Created column to access the form submission.
  2. After reviewing the information present, proceed to select New Referral. The system will automatically find & associate the form queue record to the existing referral source present in your database.
  3. The Form Queue record will be auto-dispositioned as Referral Added. Additionally, the user will be taken to a new tab displaying the existing referral source record allowing the user the opportunity to review the record and confirm any updates that occurred. 2025-02-20_16-17-55.png
  4. In the scenario above, the referral source already exists in our CRM database. Therefore, the system will simply add the new contact Lisa Doe to the existing record.

    Important: The system will adhere to the following behavior:

    • If the form submission contains a referral source & contacts already present in the system, it will not create a new record. Form Queue record will simply be linked to the existing referral source.
    • If the form submission contains a referral source and patient information, the system will create both the referral source and patient record. Additionally, it will populate the referral source as the referred-through for the patient on the auto-generated treatment opportunity.

Mapping Data Points

To see a list of the available objects that users can map to, please navigate to the following section in the CRM.

  1. CRM > Organization > Tenant > Tenant Formstack Maps.

  2. This area will contain all web forms that are integrated with your CRM tenant. Select CREATE NEW MAP to see all specific data points that are available under the Mapping From column.2023-03-15_13-57-44.png
  3. The Kipu CRM allows users to associate a 2.0 assessment template to the form stack map tied to your web form to allow you to map the additional data points directly to a matching CRM assessment form to ensure no data is missed.2023-03-15_14-00-53.png
  4. Utilizing the View icon will provide the user with a preview of the webform submission template:2023-03-15_14-03-34.png
  5. In the case that a patient is created via a form queue record containing an attached form template, the assessment will become available in the assessments section of the new patient record for your use.
