Issue
I need form data to auto-populate, or map, into the patient record from my website form.
Environment
Kipu CRM, Role: Super User, Feature: Form Queue, Tenant FormStack Maps
Resolution
Form submissions can be mapped by creating a FormStack Map within the Kipu CRM.

Visit your 3rd party form provider's account. From the form's field values area, locate and note the unique Form ID assigned along with the form keys associated with the form.
- Navigate to CRM > Organization >Tenant > Tenant FormStack Maps.
- With the Form Maps list now in view, locate the map in question by Form Stack ID or by Form Name.
- If a form map cannot be found, then one must be created by clicking Create New Map.
- Enter the required details and select Save to ensure that all the data associated with the form is accepted in the CRM.
- From the main Form Map directory, search for your newly created Form Map.
- Open the Form Map by left clicking within its corresponding field and the map will appear on the right side of the screen.
- Open the Form Map by left clicking within its corresponding field and the map will appear on the right side of the screen.
- With the form map open, select the map fields that most similarly correspond the questions, or fields, by using the available drop-down options in the Add a map field.
- Next, add the associated key for the corresponding field and select Save.
- Repeat the process as needed.
