How Can I Map Facilities to The Form Queue?

I'm interested in mapping our organization's facilities to the form queue application to auto-populate their assigned facility based on their web form submission. 

Environment

Kipu CRM, Roles: Super User 

Resolution

To map your facilities to the form queue, follow the steps provided below. If you encounter any issues through these steps, please reach out to Customer Support for additional assistance.

  1. Log in to your Wordpress account which contains the Gravity Forms integration.
  2. Navigate to your Forms page using the navigation panel on the left side of your screen.
  3. Select Edit on the form you would like to update.
  4. Click the Drop Down option in the Standard Fields section on the right and drag it to your desired question location.

  5. Choose the Desired Facility Field Label.
  6. Click the Edit Choices button.

  7. Ensure that Show Values has been selected.
  8. Enter all facility names in the Label fields and corresponding facility IDs in the Value fields.

    1. Important
      In order to locate the IDs tied to your facility records in the Kipu CRM, please navigate to CRM > Organization > Facilities. In the facility grid, reference the Facility ID column.

  9. Save Form.
  10. Navigate to the Settings tab and choose Webhooks from the drop-down menu.
  11. Find the appropriate CRM Webhook and select Edit.
  12. Using the scrollbar, scroll down to the Field Values area.
  13. Identify the last field and select the + icon on the right side to add a new field.
  14. In the left field, create a key of your choosing. No special characters are allowed. In the right field (Value), select the corresponding webform.
  15.  Save Settings.
  16. Return to the Kipu CRM. Navigate to CRM > Organization > Tenant > Tenant Formstack Maps.
  17. Locate and select the relevant Tenant Form Map tied to the webform you are configuring.
  18. Using the Add a Map Field, select the Facility ID object.
  19. Paste the key that corresponds to the facility field you added to your webform.
  20. Select Save.

Important
Please note that if a user does not have access to the expected facility, the system will display the default facility on the form queue record that the user has access to.
