Issue
I need to map the Ethnicities and Race fields from my webform submissions to the Patient Record.
Environment
Kipu CRM, Roles: Super User, Patient/Prospect Records
Resolution
To achieve this integration, follow the numbered steps provided below. If you encounter any issues through these steps, please contact CRM Support for additional assistance. The following example will showcase how to map Race. Ethnicities can be mapped following the same process.
- Log in to your Wordpress account which contains the Gravity Forms integration.
- Navigate to your Forms page using the navigation Panel on the left hand side of your screen.
- Select Edit on the form you would like to update.
- With the form open, choose the Drop Down field type on the right-hand side of the screen and drag to your desired question location.
- Choose Race Field Label.
- Select Edit Choices from the panel on the right-hand side of your screen.
- Ensure Show Values has been selected.
- Enter all Races as the Labels followed by entering corresponding Values. Unlike Labels, Values must contain no spaces.
- Save Form.
- Navigate to the Settings tab and choose Webhooks from the drop-down menu.
- Find the appropriate CRM Webhook and select Edit.
- Using the scrollbar, scroll down to the Field Values area.
- Identify the last Field and select the + icon on the right side to add a new field.
- On the left-side field, create a Key of your choosing. No special characters are allowed. On the right-side field (Value), select the field from the webform that this key corresponds to.

Save Settings.
- Return to the Kipu CRM. Navigate to CRM > Organization > Tenant > Tenant Formstack Maps
- Locate and select the relevant Tenant Form Map tied to the webform you are configuring Facility mapping for.
- Using the Add a Map Field, select the Facility ID object.
- Paste the Key that corresponds to the facility field you added to your webform.
- Select Save.
