How to Create an Assessment

The Template Designer feature allows you to easily customize templates, apply conditional logic to fields, and manage the layout as you go. This application serves the purpose of creating and managing pre-admission evaluation forms and insurance-type forms that play a crucial role in facilitating the admissions process.

 

Template Types

There are two main template types, Pre-admission and Insurance form. 

  • Pre-admission forms will only be available for selection in the assessments app for patients.
  • Insurance forms will be available in the assessments app for patients and VOB 2.0.

User Requirements

Only users with one of the following roles can create and manage assessments. 

  • Super User
  • RB Client Super User
  • RB Client Super User with User Management
  • KipuCRM Super User
  • Combined (Admissions and B.D.)
  • Combined +
  • VOB Worker
  • Restricted Admissions Worker

Creating an Assessment

Use the following steps to create an assessment.

  1. Navigate to CRM > Admissions Management > Template Designer.2025-04-17_17-01-42.png
  2. Input your desired assessment name within the Title / Name field. Next, use the Effective Date field to indicate when this assessment should be available for use.2025-04-18_09-02-19.png
  3. Select the ellipsis icon in the Add Question box.
  4. Choose your preferred question type from the list.
    • Please refer to the Question Type List article for more information on the available options.
       
  5. Select the field labeled as question 1. The right-side General panel will update to display all relevant information for that field.
  6. On the General panel, use the Name field to describe the field. This must be filled out in order to save the new field on the template as the name is used to identify the field within the JSON file. 
    • Note: Use underscores to separate words as spaces and special characters are not accepted in the name field.
  7. Next, select the Title field and input your desired field title. This can be a field label, question, etc. 
    • Note: This title will be visible when working the assessment.
  8. Next, input your desired description by selecting the Description field as needed. This will appear below the field's Title.
  9. Set the Visible, Required, and Read-only toggles to your desired configuration. You may select more than one toggle.
    • Visible : Selecting this toggle will ensure the field appears on display when working the assessment.
    • Required: Selecting this toggle will mark the question as required in order to complete the assessment.
    • Read-only: Selecting this toggle will restrict users from making any changes or edits to the field when completing the assessment. Commonly used for informational content on the assessment.
  10. Select the Input Type field and choose your desired input type
    • Note: Pick the option that corresponds to the type of information you are requesting.

  11. Repeat the steps above for each question on your assessment template. 
  12. On the left-side panel, set the Patient Signature Required, Billable, and Signatures to your desired configuration:
    1. Patient Signature Required: Enabling this toggle will trigger the form to require a patient signature once transferred to the EMR as a live form.
    2. Billable: Enabling this toggle will mark the form template as billable once transferred to the EMR as a live form. Click Here for more information on billable forms.
    3. Signatures: Utilize this dropdown field to specify which user roles will be necessary to apply a review signature when the form is transferred to the EMR as a live form.  
  13. Once you are done building out your template, select Save Changes to record your template in the system.

Previewing an Assessment

After you have finished constructing your template, see the steps below in order to preview your assessment.

  1. Navigate to CRM > Admissions Management > Template Designer.2025-04-17_17-01-42.png 
  2. Locate your assessment by inputting the name of the template within the Select an Assessment to Edit field and select the corresponding option.2025-04-18_09-02-19.png
  3. Once the template has populated on screen, select Preview.2025-04-18_09-02-19.png 
  4. A preview version of the template will appear. Users can simulate their responses to the assessment using this screen. 2025-04-23_11-33-56.png

    Important: Active CRM form templates are available for selection within the assessment section of your patient records.

Assessments as PDF Files

Assessments saved as PDFs often truncate long text unless text boxes are manually resized before saving. Follow the steps below to ensure that assessments that are saved as PDF files and sent to the EMR show full length prior to being sent. 

 

  1. Navigate Admin > Designers > Assessment Designer.

  2. Use the Select an assessment to edit field to open a previously created assessment.

  3. Select the field type called Long Text.

  4. Select the column called Layout in the side panel while highlighting the Long Text field type.

  5. Set the Expand input field dynamically to Enabled. Set the Enable resize handle field to Disabled.

    Important: The truncation occurs because the PDF reads the text box as an image, not dynamically adjusting to text length. These steps should resolve the text issue.
