How do I delete an assessment or form from a patient record?

Issue

I need to permanently delete or remove an assessment from a patient's record.

Environment

Kipu CRM, Roles: Super User, Combined Admissions, Admissions Worker, Admissions and Business Development

Resolution

To delete an assessment or form from a patient's record a user will need to navigate to the Assessments area, identify the desired assessment, followed by performing a delete action.

Important Notes

In order to delete an assessment a user must first ensure it has been disassociated from all treatment opportunities first. See article to disassociate Assessments form Treatment Opportunities here.
 

  1. Using the Global Search Bar, search for and choose the desired patient to access their record.
  2. Locate the left-hand side navigation panel as select Assessments.
  3. Identify the assessment and select the Actions icon.
  4. Choose Delete from the menu items.
  5. Select Yes to confirm you have the correct assessment or form.
