The Kipu CRM allows users to design form templates compatible with the Kipu EMR. These templates can serve various purposes, including facilitating the review process for assessments that require signatures, aiding in the representation of billable services, and auto-populating data into other EMR form templates, to improve documentation practices.
Requirements
- Must have a compatible CRM Live Form Template
- Live Forms Transfer toggle must be set to Enabled

Compatible Form Fields
The Kipu CRM Live Form Transfer feature is only compatible with certain field types. Please see the list below of all compatible form fields:

Field Set-Up Requirements
The CRM Live Forms feature requires users to set up Radio Button Group and Multi-Select Matrix fields in the way outlined below to ensure the successful population of data once transferred to the Kipu EMR.
Important
Field labels must stay under 255 characters, or the live form will fail to render in Kipu EMR.
Radio Button Group Fields
- Navigate to CRM > Admissions Management > Template Designer.
- Locate your assessment by entering the name of the template in the Select An Assessment to Edit field and selecting the corresponding option.
- Once the template appears, locate your desired Radio Button Group field.
- Click on Choices from the General panel on the right side of the screen.
- In the Choices section, use the top left field to create your first choice option. We will refer to this field as the value field.

Next, use the field in the second column to add the text that will appear on the form. We will refer to this field as the text field.
Important: The text and value fields must be different for the field to render correctly in the Kipu EMR.
- Repeat the steps above to create the remaining choices for the field.
- Once you have set up all your radio button group fields, select Save Changes.
Multi-Select Matrix Fields
- Navigate to CRM > Admissions Management > Template Designer.
- Locate your assessment by entering the name of the template in the Select An Assessment to Edit field and select the corresponding option.
- Once the template opens, locate the Multi-Select Matrix field you want to configure.
- In the General section, use the Cell Type drop-down to choose from Dropdown, Single-Line Input, or Long Text.
- Note: These are the only applicable cell types for the Live Forms Feature.
- Select Columns from the settings panel on the right.
- Then use the Name field to create your first column.

Leave the Title field as is.
- Note: The Name and Title fields must match exactly or the column will not appear correctly in the Kipu EMR.
- Repeat the steps above to build out your remaining columns for the matrix.
- Next, select Rows.
- Create your first row by entering text in the Value field.
- Next, leave the text field as is.

- Repeat the steps to build out the remaining rows.
- Now it's time to create the unique drop-down options for each column. To begin, select the first column header.
- In the General panel, click the Cell Type field and select Dropdown.
- Note: You may select either Dropdown, Single-Line Input, or Long Text.
- Next, select Choices.
- Note: Choices only need to be configured when using the Dropdown cell type.
- To add your first choice, select Add a choice.
- To create the first choice, type your desired text into the value field.
- Then enter the same text into the text field. Both the text and value fields must have exactly the same text entered.
- Repeat the steps above to create the unique dropdown field choices for each column.
- You have successfully set up a Multi-Select Matrix field compatible with the Live Forms feature. Lastly, select Save Changes to record the changes to the form template.
Important
For Dropdown fields within a matrix, the value & text fields must match in order for it to populate correctly in the Kipu EMR. Please the image below.
Transferring Live Forms to the EMR
Please follow the steps below to successfully send a live form to the Kipu EMR.

Use the Global search bar to access the patient record.
- From the patient record, use the quick panel to select Treatment Opportunities, or scroll down to Treatment Opportunities.
- From the Treatment Opportunities section, select your desired treatment opportunity record.
- In the Treatment Opportunity, click the Transfer to EMR button at the top of the screen.
- You will be presented with the list of available attachments and assessments that can be included in the transfer.
- Attachments: General PDF documents
- Assessments: CRM Live Forms
- Set the toggle to Included to add the desired CRM Live Form to the transfer.
- Select Queue Transfer to EMR.
- The selected form or forms will be sent to the EMR. For instructions on accepting incoming transfers in the EMR, click here.
- Once the incoming transfer has been accepted, the CRM live form will populate in the KipuCRM tab.
Important: Once you transfer your first live form, the KipuCRM tab will automatically generate within the EMR facesheet - Here is a preview of a CRM live form in the Kipu EMR.
