How Do I Format A Field In Assessment Designer To Support phone numbers?

Issue

I need to create a phone number field that will validate and process entries according to the standard U.S. phone number format.

Environment

Kipu CRM, Roles: Super User | Assessment Designer

Resolution

To duplicate a CRM form template, follow these steps:

1. Navigate to Admin > Designers > Assessment Designer
2. Select an Assessment to Edit from the panel on the left-hand side of the screen.
3. Choose Single-Line Input field type to add a new field.
4. Enter a desired Question Name in the General panel on the right-hand side of the screen.
5. In the General panel, scroll down to Input format.
6. Enter the standard U.S. format in the Input format field as [phone].

7. Save Changes.
