How do I configure a CRM form template to be linked to a webform integrated with my Kipu CRM, ensuring that all responses documented in the submission are displayed in the CRM?

For guidance on how to map webform fields to a CRM form template in Kipu CRM, please refer to the steps below.
- To begin, you will need to have a test form submission that you can reference in the CRM form queue to obtain the keys to the fields on the JSON. Navigate to the desired webform located on your company website.
- Using the desired webform, perform a test submission.
- Once you have completed a test submission, return to the Kipu CRM. Navigate to CRM > Admission Management > Queues > Form Queue.

Left-click anywhere on the row containing your test submission in the Form Queue grid to open the corresponding record.
- The form submission details will appear on the screen. Click the eye icon to view the payload, which contains the keys for the fields present in the JSON.
- The JSON preview will appear. Keep this page open on a separate tab or monitor as you will need to reference it for the upcoming steps.
- Using a new tab or window, Navigate to Admin > Designers > Assessment Designer.
- Use the Select an Assessment to Edit field of the Assessment Designer to search for and select the desired form template that was built to match your webform.

- The CRM form template will appear on screen.
- Select the first field present on the form template to begin mapping it to its counterpart on the webform.
- After selecting the field, the right-side panel will update to display relevant details corresponding to this field. Using the Question Name field, paste in the JSON key that corresponds to the field on your webform.
- In this case, I am pasting the "PatientF" key as that is linked to the Patient First Name field on my webform.

- Select the next field present on the CRM form template to continue the mapping process.
- Using the Question Name field, paste in the JSON key that corresponds to the field on your webform.
- In this case, I am pasting the "PatientL" key as that is linked to the Patient Last Name field on my webform.
- In this case, I am pasting the "PatientL" key as that is linked to the Patient Last Name field on my webform.
- Repeat the steps above until you have completed mapping all desired webform fields in your CRM form template.
- Once you have completed your desired mapping, select Save Changes to record your changes.
- Now, its time to link the CRM form template to the form map record. Navigate to Admin > Integrations > Form Mapping.
Important
This step is necessary to ensure that all new form submissions in the Form Queue from this webform include the CRM form template, displaying all responses to the relevant fields. It will only affect submissions made from this point forward.
- Locate and select the form map record that corresponds to the desired webform.

- Use the Select an existing Assessment field to view all available CRM form templates that are present in your CRM account.
- Select the CRM form template that you want to connect to the webform. Once applied, select Save to record your changes.
Important
Once a CRM form template is connected to the form map record for the webform, any new form submissions that appear in the Form Queue will include a view option to review all responses to the fields present on the webform. Additionally, when a form queue record is converted to a lead, the form representing the webform responses will remain accessible in the Assessments section of the patient record.
